PHI will never be sold, shared for marketing purposes, or disclosed beyond treatment, payment, or healthcare operations without your explicit written authorization, except as mandated by law.

AHBA prohibits retaliation against any individual for exercising privacy rights or filing a complaint.

  • Conduct a prompt internal investigation.
  • Implement mitigation measures to reduce potential harm.

  • Report the breach to applicable regulatory authorities when mandated.

All personnel must immediately report suspected or confirmed breaches to the Privacy Officer

This NPP is updated over time due to laws, technology, or clinic operations. The most current version will always be available on our website and at the clinic.
